2. Complete:

Fill out the absentee ballot application.

In Box 1, designate the reason for which you’re requesting an absentee ballot. New Yorkers can now designate the potential of contracting COVID-19 as a reason to receive an absentee ballot by checking the box for “Temporary Illness.”

In Box 6, designate your preference for how your absentee ballot will be delivered to you – either by mail or in person at the Board of Elections.

3. Sign:

Do not use a digital signature on this form – they will not be accepted. Please sign the form by hand.

4. Deliver via Mail or In Person:

Return the completed form either by dropping it off at your Borough Board of Elections office in person or by mailing it to:

Completed forms returned by mail must be postmarked by the United States Postal Service no later than June 7, 2021.
